Social Care

Products and services in the Social Care category help advance social determinants of health (SDoH) by identifying, tracking, and helping mitigate barriers to care, including food scarcity, financial hardship, lack of transportation, and other system issues. Functionality across these products includes:

• Identifying potential barriers to care using collected data and internal datasets, and surfacing potential issues to providers and care managers at the point of care

• Tracking patients both longitudinally and across cohorts to ensure quality of care across patient populations
